Please be sure to also fill out and sign the paper procurement form and drop it off taped to your item for record keeping/auditing purposes. All procured items are required to be completed by the FINAL procurement dates: 1/16 and 19th. 

Item information: Information about the item you are donating. Please be as detailed as possible with all item descriptions and include procured total value.

Families please select "INDIVIDUAL donor"; do not select "business donor."

Each family is responsible to procure/donate 3 items valued >$50; one item being a ready to be auctioned off gift basket.
